You are invited to participate in a study being conducted by the Appalachian Care Consortium funded by the HRSA Rural Community Opioid Response Planning Grant.
The purpose of the study is to survey the community stakeholders to identify assets and gaps in services and resources regarding the opioid issues in the tri-state/tri-county region of Bell County, KY, Claiborne County, TN and Lee County, VA. Participation should take approximately 8 minutes to complete.
PARTICIPATION Your participation in this survey is voluntary. You may refuse to take part in the research or exit the survey at any time without penalty. You may skip any question you do not wish to answer for any reason.
BENEFITS & RISKS You will receive no direct benefits from participating in this research study. However, your responses may help us learn more about However, this research is expected to increase knowledge of factors affecting opioid misuse within the tri-county region. This information will be used to develop a plan to mitigate or reduce the negative impact of such drug misuse. There are no foreseeable risks involved in participating in this study other than those encountered in day-to-day life and/or he risk that you may find some of the subject matter to be sensitive or cause emotional discomfort.
Your electronic survey answers will be stored initially with Qualtrics in a password protected electronic format. Data will later be downloaded and stored in the LMU encrypted network. No identifiers will be collected – name, email address, IP address, user name/online identifier.
